W. Carl Cooley, MD, Medical Director, is a developmental pediatrician with ten years' experience in the study of primary care office systems and strategies for change and improvement in the care of children with special health care needs. Dr. Cooley is also Medical Director at the Crotched Mountain Foundation and Crotched Mountain Rehabilitation Center in Greenfield, New Hampshire, and an associate professor of pediatrics at the Dartmouth Medical School. Prior to his fellowship training in developmental pediatrics, Dr. Cooley spent eleven years in community-based, primary care pediatric practice. Dr. Cooley lives in Concord, NH with his wife and three children, one of whom is a teen with special health care needs.

Jeanne W. McAllister, RN, MS, MHA, Director, is a project director for medical home initiatives operating out of the Center for Medical Home Improvement. She brings a background in nursing, health science professional education, and health care management and nine years experience in the improvement of primary health care systems for children with special health care needs. Her work has focused upon the development of community-based medical homes as a fundamental resource for children and families creating a community resource for schools and agencies. Prior to her work in pediatric primary care, Ms. McAllister was a clinician and an educator for children and adults with chronic health conditions. She lives in Wilmot, NH with her husband and three children.

Kathleen Sherrieb, RN, MS, MPH, supports data management at CMHI. She is a clinical nurse specialist in community health with a focus in maternal-child health. She has over 20 years of experience the development, implementation, and evaluation of care to children and their families. Over the past 10 years, she has been involved in community-based care, provider focused education, and research related to HIV/AIDS in women and children. Joining the Hood Center in 1997, Ms. Sherrieb has supported many projects; specific to CMHI she has participated in the development and validation of the Medical Home Index and the Medical Home Family Index, and is responsible for the management and analysis of data related to this project. Ms. Sherrieb has coordinated evaluation projects for several programs in the Hood Center serving children with special health care needs and their families including CMHI's Vermont and New Hampshire statewide expansion project. She is currently completing doctoral work at the Harvard School of Public Health. Ms. Sherrieb lives in Lyme, New Hampshire with her husband.

Leah Reed, Program Coordinator, Leah is the project coordinator for the Center for Medical Home Improvement (CMHI). In her role, Leah supports CMHI's medical home improvement team activities and provides coordination for research designed to demonstrate the value of the medical home for children, youth and families. Leah has seven years experience empowering caregivers whose children have serious emotional and behavioral disorders to become full partners in their care. Leah holds an Associates of Science in Human Services degree and brings experience with the delivery of community based training programs. A strong skill set includes event planning and scheduling, resource organization, and outreach to families and professionals linking them with pertinent resources. Leah is also the mother of three adult children lives with her husband and is an active artisan.
