CMARSolunteer Application	Fill out this application for the CMARS staff to review.
Division of State Police Criminal Record Release Authorization	NOTE: Please sign form in the presence of a notary.
Department of Health and Human Services Criminal Record Release Authorization	NOTE: Please sign line 6 in Section I certifying your identity . Also sign line 9 Section II of the form in the presence of a notary to authorize the release of information.
Baer	NOTE: The Baer form will need to have your signature witnessed.
